Acute Postnatal Referrals
 

Acute postnatal transfer request will normally be made by neonatal / paediatric medical staff. The cot bureau coordinator will ask the referring clinician for the following details:

  • Name of referring clinician and job title
  • Referring hospital and contact number
  • Infant's name, address and postcode
  • Infant's local hospital number or NHS number
  • Infant's date and time of birth
  • Gestation at birth & current corrected gestation
  • Infant's birth weight and current weight
  • The type of care the infant will require
  • A brief summary of the infant's current condition and reason for requesting transfer

Once these details have been provided the cot bureau coordinator can search for an appropriate neonatal cot.

A transport team clinician will then call the referring hospital back and take a full clinical history. It is therefore very helpful if all the infant's notes including gas charts, blood results, prescription charts etc are available when this call takes place. Making an Acute Postnatal Referral

  1. Inform the parents that the transfer needs to take place
  2. Ensure that medical and nursing transfer letters are completed, blood results / gas charts etc are photocopied and all x-rays are available (CD or hard copy)
  3. Telephone the cot bureau on 0161 276 8847, request the transfer and provide the required details
  4. The cot bureau will contact appropriate neonatal units in order of proximity to the referring hospital
  5. The transport SpR or ANNP will contact the referring hospital, take the clinical history and offer immediate management advice. In certain circumstances the Transport Team will mobilise at this point before a final destination has been found. This cannot however be guaranteed.
  6. When a neonatal unit is found that can accomodate the infant(s) the cot bureau will telephone the referring hospital with the appropriate details
  7. An ambulance will now be called and the team will mobilise when it arrives.
