Planned postnatal transfer request will normally be made by neonatal nurses, midwives or practitioners. The cot bureau coordinator will ask the referring clinician for the following details:
- Name of referring clinician and job title
- Referring hospital and contact number
- Infant's name, address and postcode
- Infant's local hospital number or NHS number
- Infant's DOB
- Infant's clinical status
- The type of transfer being requested
- The type of care the infant will require
- Which hospital the infant is to be transferred to
Once these details have been provided the cot bureau coordinator can schedule the transfer into the Transport Team's work allocation.
Prior to calling the ambulance the team member(s) undertaking the transfer will telephone the nurse looking after the infant for further clinical details. It may therefore be helpful to have the infant's notes, gas charts, prescription charts available when this call takes place.

Making a Planned Transfer Request
Please note: under normal circumstances at least 24 hours notice should be given for all back transfers and clinic appointments
- Telephone the accepting hospital and confirm that they still have a cot for the baby
- Inform the parents that the transfer is taking place
- Ensure that the medical and nursing transfer letters have been completed and, if appropriate, that any blood results / gas charts are photocopied and x-rays are available (CD or hard copy)
- Telephone the cot bureau on 0161 276 8847, request the transfer and provide the required details
- The cot bureau will fit the transfer into the Transport Team's work schedule
- Prior to setting out the team member(s) undertaking the transfer will contact the referring unit for further clinical details
